To register for the free assessment, simply complete the online form.

You will need the brochure given to you by your doctor. It contains a special PIN number, which is on the bottom left-hand side of the brochure. You will also require the Drug Information Number (DIN) from your CONCERTA® prescription.

After entering these two numbers in the respective fields, a coupon for a free assessment* and free first class (upon enrollment) will appear. Please print this coupon and bring it to your nearest Oxford Learning center.
